Responsibilities
In your job, you will have the ability to make a difference in a patient’s journey by:
- Perform coordination of care during the full continuum of cancer care experience.
- Identify potential and real gaps in care delivery; seek out opportunities for avoidance and/or closure.
- Prepare for, organize, and lead the Patient Care Conference (huddle) at regular intervals; communicate interventional outcomes to full care team in a timely manner.
- Perform assessment of patient risk stratification.
- Perform basic psychosocial assessments and provides emotional support; delegates and/or make referrals for additional screening and support as needed.
- Develop appropriate nursing diagnoses and corresponding action plans as needed.
- Support and/or provide oral chemotherapy education, adherence, and side effect monitoring.
- Hold or participate in survivorship and advance care planning visits; make referrals as needed.
- Participate in research, quality, and performance improvement, including reporting on navigation metrics.
- Support patient advocacy and empowerment.
- Participate in community outreach and cancer prevention education.
Minimum Qualifications:
- Bachelor of Science in Nursing or related degree is required.
- Five (5) years of work experience as a Registered Nurse in an oncology practice or clinic.
- OCN Certificate preferred.
- Knowledge of environment and systems in which patients receive care.
- Knowledgeable of practice operations guidelines, practice organization and health care economics.
- Knowledge of resources to meet patient needs, taking into consideration health literacy, culture, language, and amount of information desired.
