Hospice RN – In-Home Care | Phoenix, AZ

Full-Time

Salary: $80,000 – $95,000 annually

About the Role

Join our compassionate hospice team providing in-home care to patients and families in Phoenix. As a Hospice RN, you’ll deliver patient-centered care, manage symptoms, and support families during critical moments, all while working in a collaborative, supportive environment.

Responsibilities

  • Provide skilled nursing care in patients’ homes
  • Develop individualized care plans and manage symptoms
  • Educate and support patients and families
  • Collaborate with physicians, social workers, and other care team members
  • Maintain accurate documentation

Qualifications

  • Active RN license in Arizona (or compact-eligible)
  • Minimum 1 year of clinical RN experience (hospice/home health preferred)
  • Strong communication and critical-thinking skills
  • Compassionate, adaptable, and patient-focused
  • Valid driver’s license and reliable vehicle
